Study Population And Procedures
Consecutive patients with sore throat were assessed for inclusion in the study. Inclusion criteria were patients aged 6 years and over with three or four Centor criteria , and absence of cough), and a positive rapid antigen detection test for group A streptococcus. These criteria have been used in other studies in which the efficacy of the reference treatment has been established.223 The Centor criteria and the rapid antigen detection test for group A streptococcus are well known to primary care physicians in Sweden and have been used for several years in treatment guidelines. Before the start of the study, we did not provide any additional training about the use of Centor criteria or the rapid antigen detection test for group A streptococcus. The primary healthcare centres used the same rapid antigen detection test that they used in their normal clinical practice.
Patients were not eligible for inclusion when they showed signs of serious illness or had hypersensitivity to penicillins when they were receiving immunomodulating treatment corresponding to at least 15 mg prednisolone when they had received antibiotics for pharyngotonsillitis in the past month or when they had received any antibiotic treatment within 72 hours of inclusion. We recruited patients from 17 primary healthcare centres in urban and rural regions of Sweden: Skåne, Kronoberg, Västra Götaland, and Södermanland.

Patients at low risk of GABHS pharyngitis can be treated symptomatically, RADT should be ordered for those at intermediate risk, and empiric antibiotics are an option for those at high risk.

The American Academy of Pediatrics guidelines suggest that amoxicillin is equally effective and more palatable than penicillin for treating streptococcal pharyngitis. For children who have acute GABHS pharyngitis, amoxicillin may be a good alternative for those unlikely to comply with penicillin use because of taste, or whose parents prefer to avoid an antibiotic injection. Penicillin and amoxicillin are inexpensive and generic, and GABHS isolates have not developed resistance to these drugs.

The main treatment for strep throat is a course of antibiotics.
