Pharmacy gag rules prevent pharmacists from telling patients that they could save money on drugs, for instance by not using their health insurance.

Pharmacy benefit managers are the middlemen between drug companies and pharmacies, and some of those companies have agreements forbidding talk of discounts. But some states have also banned pharmacists from giving this information to customers.

According to the National Conference of State Legislatures, at least 22 states have some kind of gag rule legislation.

One way patients can get around this is to ask, but few people think to do so.

Ross and colleagues decided to see what would happen if Medicare patients just took advantage of Walmarts program offering $4 generic prescription drugs.

They looked at Walmarts generic list for drugs commonly used to treat heart conditions, including high blood pressure and high cholesterol.

Next, we used Medicare prescription drug plan data from June 2017 to determine beneficiary out-of-pocket costs for the lowest-priced dose of each drug in each plan, they wrote. They got data on more than 2,000 Medicare prescription drug plans, including Medicare Advantage plans.

Overall, 21 percent of the plans asked patients to pay more out of pocket for the drugs than they would pay if they just got them for $4 at Walmart, the team reported.

The $4 List: Pharmacies With Cheap Prescriptions

Money expert Clark Howard is one of many people who take advantage of $4 generic drugs available through his local pharmacy. Although the list isnt as extensive as it used to be, there are still places one likely near you that offer medications for $4 or less.

In this article, Ill focus on some major pharmacy chains that sell prescription medications at the $4 mark or cheaper.

In researching this article, I signed up for each retailers prescription savings plans where it was required to get the deal, and I learned about the cheapest medication offerings.

For most pharmacies on our list, youll find that the pricing structure provides a 30-day supply of a generic drug for $4. Of course, youll pay more for a 90-day supply.

Some big-name pharmacies, such as Walgreens, didnt make our list, although the retailer offers prescriptions as low as $5. Publix offers a basement price of $7.50 for a 90-day supply on select prescriptions.

The Walmart $4 Prescriptions program requires no membership, no fee, and no insurance, and offers patients discounted medications in 30-day and 90-day supplies. Often referred to as Walmarts $4 generic list, the program includes nearly 100 different generic medications from between $4 and $40.

Remember that your healthcare provider has to prescribe you one of these generics for you to be able to take advantage of these savings. Speak with them about your options, and ask them to opt for generic when possible.

At first glance, Wal-Marts plan to sharply cut the cost of generic drugs, to $4, seems like a signal event in American health care. It could make scores of treatments affordable to the uninsured, reduce the burden on Medicaid and bring competitive pricing to the pharmacy industry.

Even company critics have praised the plan, conceding that it represents a case of the giant retailer using its size and ability to wring out costs to improve the lives of regular Americans.

But a close examination of the program, with details confirmed by the company yesterday, suggests that its impact could be blunted by several factors.

The plan, which is said to cover 300 drugs, includes only about 124 separate medicines in various dosages, like 12 versions of the popular antibiotic amoxicillin. It leaves out some popular drugs altogether, like the generic version of the cholesterol-lowering treatment Zocor.

And while uninsured people should benefit from the program, those with insurance may save only a dollar or so, making a trip to Wal-Mart not worth their while, analysts said. In Florida, where the program will have its debut, most people on Medicaid pay nothing and may have little incentive to shop around for cheaper prescription drugs.

While I mentioned my prescription to a friend, he told me that he gets most of his prescriptions through Walmarts discounted prescription program. Interested, I did some searching, and found that Walmart, Target, and Kroger all offer discounted prescriptions.

These offers get customers into the store, and the hope is that theyll buy something else while they are there.

Ross said no patient should decide on a Medicare plan based solely on whether Walmart offers a better deal on prescriptions.

Switching plans might not be the best idea, because different plans provide different levels of coverage for doctor visits, medical procedures and other health needs.

What we are showing is there may be some ways to save some money on some drugs by going to Walmart, Ross said.

According to the Kaiser Family Foundation, about 90 percent of prescriptions filled in the U.S. are for generic drugs. Most people get health insurance through an employer, and the typical co-pay for a generic drug for a patient covered by employer-provided health insurance is $11, Kaiser found. For a brand-name drug, the average co-pay is $33.

Walmart is moving aggressively to get a big share of the U.S. health care market. Besides having large pharmacies, stores offer free health screenings and the company has said it intends to expand its locations of retail walk-in health clinics.

Walmart is also negotiating a closer partnership with health insurer Humana, including the possibility of buying it outright, according to CNBC.

Walmart and rival Amazon are squaring off in the pharmacy arena by using their customer benefit programs to serve up prescription drug savings.

Bentonville, Ark.-based Walmart late Monday unveiled Walmart+ Rx for less, a drug discount plan for customers enrolled in its nine-month-old Walmart+ membership program, aimed at competing with Amazon Prime.

And on Tuesday, Amazon Pharmacy began offering a six-month supply of selected prescription medicines for as low as $6 for Prime members. The maintenance medication offer, first reported by Bloomberg, goes up against Walmarts 15-year-old $4 generic drug program, which proved to be a game-changer in the retail pharmacy market.

Both the Walmart and Amazon drug savings benefits target uninsured and underinsured customers who pay cash for prescriptions, as well as pharmacy patients with drug coverage who are willing to pay outside their insurance plan.

Amazon Pharmacy customers who are Prime members can get maintenance prescriptions for as low as $1 monthly if they order a six-month supply.

Under Walmart+ Rx for less, members of Walmart+ can access exclusive savings on thousands of commonly prescribed medications covering a range of health needs, including heart health, mental health, antibiotics, allergies and diabetes management. Walmart said the savings averages 65% but can climb up to 85%, and some medications are available free.

Walmart Expands Discount Generics Program

Courtesy of an article in The Tennessean, we just learned that WalMart has expanded $4 discount generic drug program to offer some of those same drugs for $10 for a 90-day supply. A list of the prescription drugs covered by the program can be found here. In addition, Walmart will offer certain prescription drugs for women for $9 for a 30-day supply, such as the osteoporosis drug Fosamax and the breast cancer drug Tamoxifen. The $4 program also covers certain Over-the-Counter drugs.

WalMart isn’t the only chain store offering discount prescriptions on generics. Other stores with low-cost generic programs include :

One thing to keep in mind about all of these programs is that the numbers of generic drugs supposedly covered can seem much higher than it really is – for instance, one drug at different dosages might be listed 4 or 5 times — one for the, say, 10 mg pill, and another for the 20 mg, 30 mg, 40 mg etc. That would be counted as 4 drugs when it’s really one.

Do you have to sign up?

Youll have to join Krogers Rx Savings Club, which costs $36 a year for an individual plan and $72 for a family plan . So if you calculate in the membership, the true cost is more than $3. But if your family requires a lot of prescriptions on Krogers savings club list, this could still be a very good deal.

Once you join the club, youll find that many of Krogers discount medications go for $6, but as I mentioned, there are a select number of prescriptions that are priced at $3.

Sams Club is another retailer that offers medications starting at $4.

Do you have to sign up?

Being that Sams Club is a membership-only grocer, youll need to join if you want to take advantage of low drug prices. Sams Club costs $45 a year for regular membership and $100 a year for a Plus membership.

If you are a Sams Club member and have a Plus membership, they offer fantastic deals with a lot of generics potentially being free for you, Clark says.

Indeed, according to the retailers website, Plus members get 10 free select generic prescriptions.
