All prescribers should be aware of the range of possible adverse effects associated with antibiotics. Though perhaps less well recognized than other adverse effects, various neurotoxicities including seizure, optic neuropathy, encephalopathy, peripheral neuropathy, and exacerbation of myasthenia gravis may result from antibiotic use.1 This risk is increased in older patients, patients with impaired renal function, and patients with preexisting neurologic conditions.1

Infection and Myasthenia Gravis

The potential for antibiotics to unmask or worsen MG is particularly significant given that patients with MG may be predisposed to acquiring infectious diseases. Possible factors that could contribute to an increased risk for infection include an autoimmune disease process, muscle weakness, and possible immunosuppressive treatments.3 Most patients with MG require immunosuppressive medication such as prednisone, azathioprine, mycophenolate mofetil, or rituximab.4 Additionally, approximately 15% of patients with MG have a second autoimmune disease such as lupus or rheumatoid arthritis, which may also require immunosuppressive medications.4 Currently available immunosuppressive medications used in MG broadly suppress immune reactivity, including the ability to react against microbes.3 While the exact increase in rate of infection is not known, it is estimated that the risk for infection while on corticosteroids increases 20% to 50%.3

Plasmapheresis And Intravenous Immunoglobulin

In plasmapheresis, also known asplasma exchange, an intravenous line is used to remove antibodies from the blood. IVIG therapy is essentially an injection of a nonspecific antibody that might dial down the immune systems production of its own antibodies, much as warm air tells a thermostat to stop pumping out heat.

These treatments bring about fast but short-lived relief from MG, and are mostly used until other medications take effect, prior to surgery or for myasthenic crisis. However, some people receive regular plasmapheresis or IVIG as a supplement to immunosuppressant drugs.

Plasmapheresis is not commonly used as a long-term treatment as each treatment requires five plasma exchanges over the course of one or two weeks. This need for repeated exchanges often leads to issues with venous access.

Myasthenia gravis is an autoimmune disorder that cannot be cured. The goal of long-term treatment is to achieve a partial or total remission of symptoms with the least amount of unwanted side effects. The antibiotics that are most likely to worsen MG are usually given in the hospital to treat very serious infections. For example, some drugs like telithromycin have a boxed warning in their prescribing information that lets doctors know that people with MG should not take them.1-5

While some of these are well known to interfere with MG, some may be used with a doctors supervision if your MG is well controlled. Just be sure to tell your doctor if you notice your MG symptoms getting worse while you take these or any types of drugs.1-5

The approach of many of us has changed in the last few years, whereby we try to be more aggressive with treatment earlier based on data suggesting that being aggressive at the time of disease diagnosis pays off dividends over time in terms of more favorable patient outcomes, he said.

He said there also is growing data that patients who reach certain favorable milestones earlier, such as within the first year or two after diagnosis, tend to have a more stable course long term, perhaps for decades, he said.

He predicts that another treatment approach that will become more common is the use of the newer drugs, such as eculizumab and efgartigimod, as bridge therapies.

These newer agents that work relatively quickly will be used to bridge patients while we await conventional oral therapies such as corticosteroids and the so-called steroid-sparing immunosuppressants, to take hold, he said.

Dr. Guidon said that as treatment options for MG continue to expand, it is going to become easier and easier to escalate treatment and we want to make sure the diagnosis is correct. She said that patients may be experiencing some symptoms not necessarily caused by MG, and it is important to establish whats in the myasthenia bucket and what is outside of it.

What Should I Discuss With My Healthcare Provider Before Taking This Medicine

You should not take this medicine if you are allergic to amoxicillin , clarithromycin , or lansoprazole , or if:

  • you have a history of long QT syndrome or certain heart rhythm disorders
  • you have ever had liver problems or jaundice caused by taking clarithromycin
  • you are allergic to azithromycin , erythromycin, or telithromycin
  • you have had a severe allergic reaction to a penicillin antibiotic, including ampicillin, Augmentin, Principen, Timentin, Trimox, and others or
  • you have had a severe allergic reaction to a cephalosporin antibiotic such as cefdinir, cefprozil, cefuroxime, cephalexin, Duricef, Omnicef, Cefzil, Keflex, Spectracef, and others.

Some medicines can cause unwanted or dangerous effects when used with amoxicillin, clarithromycin, and lansoprazole. Your doctor may change your treatment plan if you also use:

  • cisapride
  • osteoporosis or low bone mineral density or
  • an electrolyte imbalance or metabolic disorder.

You may be more likely to have a broken bone while taking this medicine long term or more than once per day. Talk with your doctor about ways to keep your bones healthy.

This medicine may harm an unborn baby. Use effective birth control to prevent pregnancy, and tell your doctor if you become pregnant.

Amoxicillin can make birth control pills less effective. Ask your doctor about using a non-hormonal birth control to prevent pregnancy.

You should not breast-feed while using this medicine.

    I am also a 60 year old male diagnosed with myasthenia gravis. I have a fairly manageable case that has improved somewhat over the last few years by reducing stress and more sleep as a result of retirement.
