For many patients with blood cancers, stem cell transplantation from a family member or from an unrelated donor remains the only potentially curative option. Unfortunately, up to 40% of patients develop chronic lung disease after the transplant, which substantially increases the risk of death in the long-term. Currently, patients with transplant-related lung disease are treated with some combination of steroids and other immunosuppressant drugs, but only about 1 out of 5 improve.

This is a pilot study. Once feasibility is established, the investigators will seek to expand this study into a definitive clinical trial.

What Are The Risks

Z-paks have been around long enough that the drug is known to be well tolerated by most patients, however, it does carry some risks.

According to the U.S. Food and Drug Administration, the pills can cause abnormal changes in the heart’s electrical activity that may lead to a fatal heart rhythm.

A benefit versus risk analysis should be performed on patients with existing QT interval prolongation, elderly patients and patients with a history of cardiac disease, Dr. Kobic said. But, overall the risk of ventricular arrhythmia is very low and reported as less than 1% in clinical trials.

When Is It Appropriate To Prescribe A Z

While your Z-pak wont work on viruses, such as colds, the flu or runny noses and even some bacterial infections, including most cases of bronchitis, many sinus infections and some ear infections, it does play a role in treating certain bacterial infections.

It is still used today to treat atypical bacterial pneumonia, chlamydia and/or gonococcal infections and is commonly used for Mycobacterium avium complex prophylaxis in HIV patients with CD4 counts below 50, Dr. Kobic said.

Percent Higher Risk Of Cardiac Events

Researchers at the University of Illinois Chicago have found that taking azithromycin could dramatically increase your risk of suffering a cardiac event if youre taking medications that alter the electrical functioning of your heart.

In fact, the scientists found that If taken together with drugs that affect the electrical impulses of the heart, the combination is linked with a 40 percent increase in cardiac events, including fainting, heart palpitations and even cardiac arrest.

The drugs that azithromycin doesnt play well with include QT-prolonging drugs like:

  • Blood pressure medications such as ACE inhibitors and beta-blockers
  • Some antidepressants
  • Anti-malaria drugs such as hydroxychloroquine and chloroquine
  • Opioid medications
  • Muscle relaxers

Disturbingly, previous research has shown that one in five people prescribed azithromycin was also taking one of these QT-prolonging drugs.

Thats a whopping 20 percent of people being given a drug that could cause them to pass out, suffer problems with their heart rhythm or even have a heart attack or die!

How To Take Azithromycin

Use Azithromycin exactly as directed on the label, or as prescribed by your doctor. Do not use in larger or smaller amounts or for longer than recommended.

Follow all directions on your prescription label and read all medication guides or instruction sheets. Use the medicine exactly as directed.

Wait Remind Me What Z

A Z-Pack is literally just azithromycin, a standard antibiotic introduced in 1992, formulated in a package of six pills, which are taken over five days.

That short time commitment was a huge part of their appeal. Other antibiotics may require multiple doses a day for 10 or 14 days at a time, Tara Vijayan, M.D., assistant clinical professor in the Division of Infectious Diseases at the David Geffen School of Medicine at UCLA, tells SELF. The Z-Pack was a nice defined course and relatively short, she says. It made people feel like they can do this they can complete their course and theyll be done with it.

Plus, the ones I took were usually hot pink and came in a blister pack rather than a bottlefactors that mattered a lot to a sick and miserable 8-year-old me. They were cheap, easy, cute, and they had a little marketing zing, Neha Vyas, M.D., a family medicine physician at Cleveland Clinic, tells SELF.

And, Dr. Vyas says, theyre indicated for a bunch of different health issues , which made them an appealing option when the first-line treatments werent available. For instance, before azithromycin, if you had a penicillin allergy and needed an antibiotic for a case of strep throat, your only option was likely a cephalosporin antibiotic, which still has some cross-reactivity with penicillin allergies, Dr. Vyas explains. So, if there were any worries about allergies, a Z-Pack was an obvious, nearly all-purpose choice.

How to pronounce methylprednisolone (Medrol) (Memorizing Pharmacology Flashcard)

According to the National Institutes of Healths COVID-19 Treatment Guidelines, multiple high-quality studies suggest that corticosteroid therapy improves outcomes in people with COVID-19 who require oxygen therapy. This may be because they reduce the inflammatory reaction that leads to lung injury and dysfunction.

The most widely studied corticosteroid for treating COVID-19 is dexamethasone. Researchers have also studied other steroids like hydrocortisone and methylprednisolone, but evidence to back the use of these medications isnt as strong.

Zithromax Effectiveness In Clinical Trials

In clinical trials, Zithromax was effective at fighting bacterial infection, including some antibiotic-resistant strains.

Studies conducted before approval of the drug measured its minimum inhibitory concentration in relation to a host of bacteria. MIC is the lowest concentration of an antibiotic that will inhibit the growth of bacteria and thereby kill them. A lower MIC means a more effective antibiotic.

In a 1991 study in the European Journal of Clinical Microbiology and Infectious Diseases, researchers found Zithromax had a markedly low MIC against some bacteria compared with three other types of antibiotics, meaning it was highly effective for example, resolving 92 percent of gonorrhea infections treated.

What Is The Dosage For Azithromycin

  • Azithromycin can be taken with or without food, but food reduces stomach upset.
  • Zmax should be taken on an empty stomach 1 hour before or 2 hours after a meal since food reduces its absorption.

  • For most infections, azithromycin is taken once daily for a relatively short course of treatment .
  • The first dose is often a “double dose,” twice as much as the remainder of the doses given.

Treating Strep Throat With Z

If your doctor thinks azithromycin is a good choice for you, they may prescribe a generic version of azithromycin or a Z-Pack.

Each Z-Pack contains six 250-milligram tablets of Zithromax. You would take two tablets on the first day, followed by one tablet daily for four days.

A Z-Pack typically takes at least five days to fully work, but it can start to relieve your sore throat and other symptoms on the first day you take it. If your doctor prescribes a generic version of azithromycin, your treatment may only last three days.

Be sure to take your Z-Pack or generic azithromycin exactly as your doctor prescribes. This is true even if you feel better before youve taken the full course of treatment.

If you stop taking an antibiotic early, it could make the infection come back or make future infections more difficult to treat.

Why Is This Medication Prescribed

Azithromycin also is used to treat or prevent disseminated Mycobacterium avium complex infection . Azithromycin is in a class of medications called macrolide antibiotics. It works by stopping the growth of bacteria.

Antibiotics such as azithromycin will not work for colds, flu, or other viral infections. Using antibiotics when they are not needed increases your risk of getting an infection later that resists antibiotic treatment.

Theres More Nuance To Antibiotic Prescribing Than You Might Realize

The way doctors think about prescribing all antibioticsnot just Z-Packshas evolved since these medications were introducted.

First off, both of the experts SELF spoke to for this story emphasize that what may seem like a penicillin allergyor, more likely, what youve been told all your life is a penicillin allergymay not be a true allergy. Many people, like me, grow up being told that they had some kind of a rash or other vaguely bad reaction after getting penicillin treatment as a baby, which is very possibly true. But people grow out of these sorts of reactions, Dr. Vijayan says, and doctors now appreciate just how exceptionally rare true penicillin allergies are.

About 10 percent of patients report a penicillin allergy, according to estimates from the American Academy of Allergy, Asthma, and Immunology , but about 90 percent of them may not actually have one. So, many of the people who took Z-Packs because they thought it was their only option may have been perfectly fine just getting the penicillin instead, making them even more egregiously overprescribed.

Weve also come to understand that there are shades of gray to the conventional wisdom of taking every last dose of your antibiotic, even if you feel better long before that, Dr. Vijayan says. The truth is that, for many antibiotics that are routinely prescribed in primary care, a shorter course is totally finesometimes even betterthan a longer one.
