Systematic Risks From Chloramphenicol Eye Drops
A recent paper criticised the use of topical antibiotics for acute infective conjunctivitis, with which I would strongly agree. However, there was no mention of an important contraindication to chloramphenicol eye drops: the risk of systematic complications from absorption into the general circulation of the drug through the conjunctival, nasal, and naso-pharyngeal mucosae. That would be expected anyway on obvious logical grounds. Indeed, one should always consider possible systematic effects from any and every topical applications, particularly in children, and pregnant and lactating women. Of course, chloramphenicol is very rarely used systematically because of the risk of toxicity.
As a result of a previous paper, I reviewed, in detail, the evidence of systematic toxicity from chloramphenicol eye drops. Another very interesting report has recently been published of a patient suffering acute hepatitis probably from these eye drops: the authors also mention a notification to the Committee on Safety of Medicines of two possible cases of hepatitis associated with chloramphenicol, one of which resulted from eye drops in an infant.
My clinical practice was to prescribe the antiseptic brolene , the active constituent of golden eye drops and ointment, in strong preference to any antibiotics, especially of course chloramphenicol.

Antibiotic Eye Drops For Pink Eye Patients
A majority of pink eye patients are prescribed antibiotic eye drops, though they are rarely needed to treat this common eye problem.
Pinkeye affects more than 5 million people in the United States every year. There are three types of pink eye condition: viral, bacterial and allergic conjunctivitis. Most cases of pink eye are caused by viral infections or allergies, which do not respond to antibiotics. Artificial tears and warm compresses and other treatments as mentioned above may help relieve the symptoms of pink eye.
Antibiotics are often not necessary even for bacterial conjunctivitis because most cases of bacterial conjunctivitis are mild and resolve on their own within seven to 14 days even without any treatment.
Patients are generally unaware of the harmful side effects of antibiotics and may falsely believe that these eye drops are necessary for the treatment of conjunctivitis. Lack of education is the major reason for this false belief. Educating patients about conjunctivitis and problems with self-medication may help to dispel such misconceptions about the condition and the use of antibiotics for pink eye.

Other steps you can take at home are applying warm compresses and avoiding wearing contact lenses until you no longer have symptoms. If your conjunctivitis is caused by allergies, over-the-counter allergy medicines and eye drops may also help.
If your pink eye symptoms worsen or dont get better, or if you have eye pain, intense redness in your eyes or light sensitivity, its a good idea to see a doctor who can diagnose the infection and suggest a more targeted treatment plan. Depending on the cause of your pink eye, this could include prescription antibiotic eye drops or ointment, or antiviral medication.

How And When To Use Chloramphenicol
Chloramphenicol comes in different strengths. How you apply the medicine depends on what you’re taking it for and whether you’re using the eye drops, eye ointment or ear drops.
If you’re using 0.5% chloramphenicol eye drops, put one drop into the affected eye every 2 hours for the first 2 days. Then every 4 hours for the next 3 days, or as your doctor advises.
If you’re using 1% chloramphenicol eye ointment, apply it to the affected eye every 3 hours . Do this 3 to 4 times a day, or as your doctor advises.
With more severe infections, your doctor may recommend using eye ointment at bedtime and eye drops during the day. This is because the ointment sticks to the surface of the eye and eyelid and works while you sleep. The eye drops allow you to get on with your day as normal, as they don’t affect your eyesight.
If you’re using 5% or 10% chloramphenicol ear drops, put 3 drops into the affected ear 2 to 3 times a day, or as your doctor advises.
To use the medicine:
- always wash your hands before using chloramphenicol – this prevents you from passing the infection to the other eye or ear
- remove the cap just before using your medicine and replace it as soon as you’ve finished
- do not touch the nozzle of the bottle or tube with your fingers
- make sure the nozzle doesn’t touch your eye or ear, or your skin
- use a mirror to help you see what you are doing
- tilt your head back or to the side to put drops in your eyes or ears
How Do I Know If I Need Chlorsig Eye Drops Or Eye Ointment
