Background. Dermal piercings have recently become a fashion symbol. Common complications include hypertrophic scarring, rejection, local infection, contact allergy, and traumatic tearing. We report a rare case of Mycobacterium fortuitum following a dermal piercing and discuss its medical implications and treatments. Case. A previously healthy 19-year-old woman presented complaining of erythema and edema at the site of a dermal piercing on the right fourth dorsal finger. She was treated with a 10-day course of trimethoprim-sulfamethoxazole and one course of cephalexin by her primary care physician with incomplete resolution. The patient stated that she had been swimming at a local water park daily. A punch biopsy around the dermal stud was performed, and cultures with sensitivities revealed Mycobacterium fortuitum. The patient was treated with clarithromycin and ciprofloxacin for two months receiving full resolution. Discussion. Mycobacterium fortuitum is an infrequent human pathogen. This organism is a Runyon group IV, rapidly growing nontuberculous mycobacteria, often found in water, soil, and dust. Treatment options vary due to the size of the lesion. Small lesions are typically excised, while larger lesions require treatment for 26 months with antibiotics. We recommend a high level of suspicion for atypical mycobacterial infections in a piercing resistant to other therapies.

How Painful Is The Procedure

Like any surgical procedure, there is pain involved. Most patients comment that they have a mild to moderate amount of pain. Most of the pain seen with breast lifts/mastopexy is due to the bruising and the swelling involved. In fact, most complain more about the soreness and aching sensation than actual pain. To help with the pain, we regularly give patients oral and intravenous pain medicines in the post-operative period. Upon their discharge from the hospital, we supplement this with oral pain medication. It must be noted here that at the time of the surgery, the incisions are infiltrated with local anesthetic medication that lasts six hours, in an effort to help with post-operative pain.

How Do I Know My Piercing Is Infected

It is normal to experience some level of pain immediately after the procedure. However, the pain should reduce after a couple of days. If it lasts longer than a few days, it is likely to develop a complication or infection. This is where you need to take the right mitigation measures to help the infection heal and go away.

It is normal to experience some kind of inflammation and soreness. The nipple will be red and painful to touch up to a few hours after the piercing procedure. Although a little itching may also be experienced, it should not be severe or too much. Just avoid scratching or touching it to avoid an infection.

However, if there is persistent pain, soreness, and inflammation, that is how you know your nipple piercing is getting infected. There may be more signs to look out for.

Can I Combine This Procedure With Any Other Procedure

Yes and no. Like any surgical procedure, there are limits to what can be done and what is safe. As for the yes, it can be combined with some other procedures. For example, I routinely do this procedure with a breast augmentation. I feel that this allows the breast contour to be maximized. Let me explain. Once a nipple has been lifted, it tends to sit on a hollowed area, even though it is more in the right position. Patients often complain of having poor upper pole fullness. By using an implant, the breast tissue pocket is maximized, the breast is fuller and perkier and then the new nipple positioning is ideal.

Besides an augmentation, other procedures are routinely done along with breast lifts. Any procedure that may be too long or too arduous and place the patient at risk for significant bleeding or significant anesthetic exposure would have to be deferred and done in two stages. Please understand I say this without knowing exactly what you, the patient, may have in mind. I can only speak from my experience.

Patients with acute mastitis and abscess often represent a diagnostic and therapeutic challenge. There is currently no consensus on optimal management strategies. The principles that guide successful diagnosis, evaluation, and management are discussed presently.

PRESENTATION

Patients with acute breast infection typically present with one or more of the following symptoms: skin erythema, palpable mass, tenderness, fever, and/or pain.13 Breast abscesses may occur in both the lactational and nonlactational setting.48 Breast abscesses have been most commonly reported to occur in women between 20 and 50 years of age.4,5,7,9 However, breast infections have also been reported in men,9 postmenopausal women,10 and children.11 Postmenopausal patients with breast abscess often have a more indolent presentation and may lack many of the classical findings for breast abscess.10 Abscesses may occur either centrally or peripherally in the breast. Many authors have reported that a subareolar abscess is more common in nonlactating women,1,4,7 and lactational abscesses are more common in the upper outer quadrant.5,12 Conversely, two groups have reported that abscesses in nonlactating women occur in the periphery of the breast.5,13
