Who Will Prescribe The Treatments

The FDAs emergency use authorization for the medicines notes that physicians, advanced practice registered nurses and physician assistants who are licensed or authorized under state law to prescribe these types of drugs can do so.

Pharmacists themselves will not be able to prescribe the oral antiviral pills. Rather, the test-to-treat sites have health care providers available who can assess and discuss relevant treatment options and prescribe the drugs, according to HHS.

In a statement Friday, the American Medical Association noted concern about allowing pharmacy-based clinics to prescribe antivirals to anyone with Covid-19 who comes in for testing, because the clinic might not know the patients full medical history or other medications theyre taking, as a primary care physician would.

But on Saturday, the AMA said in another statement that the Biden administration has reassured the group that patients who have access to a regular source of care should contact their physician shortly after testing positive for COVID-19 to assess their treatment options.

As the supply of antiviral medications for COVID-19 increases rapidly in the coming weeks and months, the AMA will continue to work to ensure that all patients, particularly historically marginalized groups, have safe and timely access to appropriate treatments.

Paying For Medicines Up Front Or Afterwards

If your pharmacist has a contract with your health insurer on providingpharmaceutical services, the bill will go directly to your health insurer.

If there is no such contract, you will have to pay for the medicinesyourself, but you will be able to claim these costs from your insurer later. Youcan also choose to go to another pharmacy that does have a contract with yourhealth insurer, but you will need a new prescription.

The Southern supermarket chain Publix announced today that it will be offering seven common prescription antibiotics for free in its stores, located in Flora, George, South Carolina, Alabama and Tennessee. The move was reported in an AP story and in the Palm Beach Post.

The move follows on the heels of similar moves by other retailers, to offer selected generic drugs at low cost. Last year, for instance, Wal-Mart annoucned that it would offer a list of certain generic drugs at $4 per prescription.

What’s wrong with offering consumers free drugs? After all, many people, particularly people without insurance, have a hard time paying for prescription drugs. So free drugs will help them, won’t they?

It is true that free or discounted prescriptions can help patients, particularly the uninsured and low-income. But there is something peculiar and potentially disturbing about Publix’s move: The selection of antibiotics as the drugs to be offered for free.

Antibiotics are typically used for short periods of time to treat acute infections. Unfortunately, antibiotics are also massively overprescribed, which leads to the increase in infections which are resistant to common antibiotics. The Palm Beach Post raised this concern in its article on the announcement:

Publix Super Markets has filled the 100 millionth prescription under its free medication program.

Lakeland, Fla.-based Publix announced the pharmacy milestone on Thursday. Launched in 2007, Publix Pharmacys free prescription drug program offers 14-day supplies of selected generic antibiotics and 90-day supplies of generic maintenance medications for hypertension and diabetes at no charge.

Publix noted that theres no limit on the number of free prescriptions, and the medications are provided at no charge regardless of a patients insurance coverage.

Almost all pharmaceutical manufacturers have programs to help people without insurance obtain medication at no charge, says Rich Sagall, M.D., president of NeedyMeds, a nonprofit that tracks drug discount programs and connects people with the programs free of charge.

Sagall and his team track all manufacturer discount programs, including whats known as Patient Assistance Programs, where if you can prove you earn less than a given amount, companies will send you medication free.

He says several companies, including Eli Lilly, Merck, and Novo Nordisk, have increased the maximum annual income levels for people applying to some of their programs, so you can earn more and still qualify.

Another nonprofit service, RxAssist, has a lookup tool and links to drug company assistance programs.

Manufacturer Copay Assistance Cards

Many name brand drugs manufacturers have created programs to entice patients to try and stay on their medications even if their copays through their insurance is very high. The programs work like a secondary insurance that pays for most or all of the remaining copay after your insurance is applied.

These programs are drug specific and can be found by visiting the drugs website online or googling the drugs name with the phrase copay card.

Who Can Receive Test

There are two antiviral pills authorized by the US Food and Drug Administration to treat Covid-19 at home: Pfizers Paxlovid for people 12 and older, and Mercks molnupiravir for adults. Anyone in those age groups who tests positive for Covid-19 could be prescribed the medications to take at home.

The treatments work best when a person takes them within a few days of symptoms starting or testing positive even if their symptoms are mild.

Only one positive Covid-19 test is needed for treatment, Inglesby said, and it does not have to be a PCR test: Any positive test should be sufficient.

Getting A Prescription To The Pharmacy

Ogilvie said for patients given a physical prescription, many pharmacies have an app, online or telephone service that allows you to send it to the pharmacy without going to the store.

You still do need to have the original prescription brought in to the pharmacy when you go to pick it up, but it saves you having to make multiple trips and unnecessary exposures, he said.

He said the best case scenario is for the physician to fax the prescription directly to the pharmacy.

Draper said for those using the Shoppers Drug Mart virtual-care system, the doctor will directly fax the prescription to the pharmacy of your choice, adding that it does not have to be a Shoppers Drug Mart.

Who Can Get Free Prescriptions

You can get free NHS prescriptions if, at the time the prescription is dispensed, you:

  • are 16 to 18 and in full-time education
  • are pregnant or have had a baby in the previous 12 months and have a valid maternity exemption certificate
  • have a specified medical condition and have a valid medical exemption certificate
  • have a continuing physical disability that prevents you going out without help from another person and have a valid medical exemption certificate
  • hold a valid war pension exemption certificate and the prescription is for your accepted disability
  • are an NHS inpatient

You’re also entitled to free prescriptions if you or your partner receive, or you’re under the age of 20 and the dependant of someone receiving:

If you’re entitled to or named on:

  • a valid NHS certificate for full help with health costs

Does The Program Have Restrictions

Walmarts program does have some restrictions. Its important to be aware of theseand the possibility that your medication could be removed from the list at any timebefore asking your healthcare providers to make any changes to your prescriptions.

  • Pricing may depend on where you live. In the fine print, Walmart notes that prices of some of the drugs included on the list may be higher in some states, including, but not necessarily limited to, CA and MN. If youre unsure if your state may have different pricing, call the pharmacy ahead of time to check.

  • North Dakota is not included. Walmart $4 Prescription program is available at all Walmart retail pharmacies except for those located in North Dakota.

  • Your prescription may not always cost $4. Though its explicitly advertised as the Walmart $4 Prescriptions program, your medication may not always be $4. Certain medications are $9 for a 30-day supply, and some are $10 or $24 for a 90-day supply. Confirm your pricing before you head to the pharmacy so theres no confusion when you pick up your medication.

  • Brand drugs arent covered. The program only includes generic medications.

  • Only refills are eligible for Walmart Mail Service. If youre filling a prescription from this list for the first time, you must pick it up in the store. Mail service is only an option for refills.
