At first glance, Wal-Marts plan to sharply cut the cost of generic drugs, to $4, seems like a signal event in American health care. It could make scores of treatments affordable to the uninsured, reduce the burden on Medicaid and bring competitive pricing to the pharmacy industry.
Even company critics have praised the plan, conceding that it represents a case of the giant retailer using its size and ability to wring out costs to improve the lives of regular Americans.
But a close examination of the program, with details confirmed by the company yesterday, suggests that its impact could be blunted by several factors.
The plan, which is said to cover 300 drugs, includes only about 124 separate medicines in various dosages, like 12 versions of the popular antibiotic amoxicillin. It leaves out some popular drugs altogether, like the generic version of the cholesterol-lowering treatment Zocor.
And while uninsured people should benefit from the program, those with insurance may save only a dollar or so, making a trip to Wal-Mart not worth their while, analysts said. In Florida, where the program will have its debut, most people on Medicaid pay nothing and may have little incentive to shop around for cheaper prescription drugs.

The story was similar for personal care and beauty products: Target and Walmart were much cheaper than the other stores in this category, although Kroger mounted stronger competition. CVS was priciest by a landslide, charging nearly 50% more for the entire list of 19 items than Target, the cheapest option. Again, price differences on individual products were substantial. Shoppers could pay between $24 and $27 for a tiny tube of Mederma scar treatment at CVS, Rite Aid, or Walgreens the same product cost about $15 at Target or Walmart. Kroger tended to be on the pricier side for specialty skin care treatments like the Mederma, and for cosmetics, but more competitive for everyday personal care items like soap and toothpaste.

Drugstore Chains Vs Big
Cheapism compared out-of-pocket prices at the countrys largest chain pharmacies CVS Pharmacy, Rite Aid, Walgreens, Walmart, Target*, and Kroger. Heres what we found:
- Consumers looking for the cheapest pharmacy are better off shopping in the pharmacy departments at big-box and grocery stores than at the big-name pharmacy chains.
- In our survey of prescription drug prices, Walmart and Kroger undercut the major drugstores by a wide margin. Walmart is also the cheapest place to buy many over-the-counter medications.
- When we compared prices on a range of vaccinations, including seasonal flu shots, Walmart was, again, the cheapest option.
- For personal care and beauty products, Target has the edge.
- Stand-alone pharmacies do offer some perks. Convenience, additional amenities, selection, rewards programs, and customer service make chain drugstores competitive despite higher prices.

Healthmedicare Spending On Name
These offers get customers into the store, and the hope is that theyll buy something else while they are there.
Ross said no patient should decide on a Medicare plan based solely on whether Walmart offers a better deal on prescriptions.
Switching plans might not be the best idea, because different plans provide different levels of coverage for doctor visits, medical procedures and other health needs.
What we are showing is there may be some ways to save some money on some drugs by going to Walmart, Ross said.
According to the Kaiser Family Foundation, about 90 percent of prescriptions filled in the U.S. are for generic drugs. Most people get health insurance through an employer, and the typical co-pay for a generic drug for a patient covered by employer-provided health insurance is $11, Kaiser found. For a brand-name drug, the average co-pay is $33.
Walmart is moving aggressively to get a big share of the U.S. health care market. Besides having large pharmacies, stores offer free health screenings and the company has said it intends to expand its locations of retail walk-in health clinics.
Walmart is also negotiating a closer partnership with health insurer Humana, including the possibility of buying it outright, according to CNBC.

Amazon and Walmart are ramping up their efforts to grab market share in the $360 billion prescription drug market in the US.
Today, Amazon announced that Prime members can receive a six-month supply of several widely prescribed drugs, starting at $6. Many drugs are pricier but are still discounted relative to typical cash prices. And yesterday, Walmart said it would be adding new discounts of up to 85 percent on prescription drugs purchased through its Walmart+ RX service, with an average savings of about 65 percent.
The new announcements come as the retail behemoths have largely failed to disrupt the traditional pharmacy industry. Big names still dominate, with CVS and Walgreens topping the rankings by revenue. Walmart, which has a significant physical footprint for its pharmacies, comes in fifth.
That could be why a report last month from Business Insider said that Amazon was looking at opening traditional pharmacies inside its more than 350 Whole Foods locations. But nothing has come of the idea so far, as starting a physical pharmacy is expensive. Each new Whole Foods location would need about three pharmacists and five technicians, in addition to changes in store layouts.

Walmart Drug Program Cheaper For Many Medicare Patients
Walmarts $4 generic prescription drug program ends up being cheaper for some Medicare patients than their own health insurance, according to a new study released Monday.
Its more evidence that patients cannot always rely on their health insurance to get them the lowest prices for their prescription drugs, said Dr. Joseph Ross of the Yale School of Medicine, who led the study.
Patients were paying more out of pocket when they were using their insurance than when they went to Walmart, Ross told NBC News.
The study, published in the Annals of Internal Medicine, documents that Walmart provides a better deal than the governments health insurance plan for people over 65. And that is bad news for Medicare, because if people dont take their drugs, whether for cost or for other reasons, they tend to get sicker and then end up costing even more to treat.
Everyones talking about pharmacy costs these days, Ross said. We did this study in part because of all the discussion about pharmacy gag rules.
