Warfarin And Nonsteroidal Anti
Nonsteroidal anti-inflammatory drugs increase gastric irritation and erosion of the protective lining of the stomach, potentially leading to the formation of gastrointestinal ulcers, which could result in gastrointestinal bleeding. These effects in combination with the anticoagulant effect of warfarin increase the risk of bleeding without affecting the INR.18,19 The cyclooxygenase-2 inhibitor, celecoxib , does not affect platelet function and is associated with less gastrointestinal erosion. Literature correlating gastrointestinal bleeding risk with celecoxib use is limited. Patients should be educated to report abdominal pain or signs and symptoms of gastrointestinal bleeding to their physician when taking warfarin concomitantly with any NSAID.2022
If concomitant therapy with warfarin and aspirin/dipyridamole is necessary, the dosage of aspirin/dipyridamole should be limited to 100 mg per day or less to minimize bleeding risk.18 It is preferable to use alternatives to NSAIDs, such as acetaminophen, topical therapy, or judiciously prescribed opioid analgesics when indicated. Acetaminophen does not alter platelet function, but concomitant use may increase the pharmacodynamic effect of warfarin, increasing the INR. To be cautious, limit the acetaminophen dosage to 2 g per day for no more than seven days and complete more frequent INR testing.18

Carcinogenesis Mutagenesis Impairment Of Fertility
Carcinogenesis Tadalafil was not carcinogenic to rats or mice when administered daily for 2 years at doses up to 400 mg/kg/day. Systemic drug exposures, as measured by AUC of unbound tadalafil, were approximately 10-fold for mice, and 14- and 26-fold for male and female rats, respectively, the exposures in human males given Maximum Recommended Human Dose of 20 mg.
Mutagenesis Tadalafil was not mutagenic in the in vitro bacterial Ames assays or the forward mutation test in mouse lymphoma cells. Tadalafil was not clastogenic in the in vitro chromosomal aberration test in human lymphocytes or the in vivo rat micronucleus assays.
Impairment of Fertility There were no effects on fertility, reproductive performance or reproductive organ morphology in male or female rats given oral doses of tadalafil up to 400 mg/kg/day, a dose producing AUCs for unbound tadalafil of 14-fold for males or 26-fold for females the exposures observed in human males given the MRHD of 20 mg. In beagle dogs given tadalafil daily for 3 to 12 months, there was treatment-related non-reversible degeneration and atrophy of the seminiferous tubular epithelium in the testes in 20-100% of the dogs that resulted in a decrease in spermatogenesis in 40-75% of the dogs at doses of 10 mg/kg/day. Systemic exposure at no-observed-adverse-effect-level for unbound tadalafil was similar to that expected in humans at the MRHD of 20 mg.
Cialis 5 Mg For Once Daily Use For Ed And Bph
The efficacy and safety of Cialis for once daily use for the treatment of ED, and the signs and symptoms of BPH, in patients with both conditions was evaluated in one placebo-controlled, multinational, double-blind, parallel-arm study which randomized 606 patients to receive either Cialis 2.5 mg, 5 mg, for once daily use or placebo. ED severity ranged from mild to severe and BPH severity ranged from moderate to severe. The full study population had a mean age of 63 years and was 93% White, 4% Black, 3% other races 16% were of Hispanic ethnicity. Patients with multiple co-morbid conditions such as diabetes mellitus, hypertension, and other cardiovascular disease were included.
In this study, the co-primary endpoints were total IPSS and the Erectile Function domain score of the International Index of Erectile Function . One of the key secondary endpoints in this study was Question 3 of the Sexual Encounter Profile diary . Timing of sexual activity was not restricted relative to when patients took Cialis.
The efficacy results for patients with both ED and BPH, who received either Cialis 5 mg for once daily use or placebo are shown in Tables 21 and 22 and Figure 8.

Tadalafil belongs to a group of medications called phosphodiesterase type 5 inhibitors. It is used to treat erectile dysfunction . It helps to achieve and maintain an erection sufficient for sexual activity to occur. It does this by allowing a greater flow of blood into the penis when a man taking the medication is sexually aroused. It works only with sexual stimulation and does not increase sexual desire.
Tadalafil is also used to treat symptoms of benign prostatic hyperplasia in men with enlarged prostates. It helps to relieve symptoms such as the frequent need to urinate, painful urination, or the sensation of not completely emptying the bladder. It does this by relaxing muscles in the prostate and bladder. Tadalafil may be used to treat men who experience both erectile dysfunction and BPH.
This medication may be available under multiple brand names and/or in several different forms. What Should I Discuss With My Healthcare Provider Before Taking Tadalafil
You should not take tadalafil if you are allergic to it, or:

Do not take tadalafil if you are also using a nitrate drug for chest pain or heart problems. This includes nitroglycerin, isosorbide dinitrate, and isosorbide mononitrate. Nitrates are also found in some recreational drugs such as amyl nitrate, butyl nitrate or nitrite . Taking tadalafil with a nitrate medicine can cause a sudden and serious decrease in blood pressure.
Some tadalafil can remain in your bloodstream for 2 or more days after each dose you take . Avoid nitrate use during this time.

- a stomach ulcer or
- health problems that make sexual activity unsafe.
A small number of people taking tadalafil have had sudden vision loss. Most of these people already had eye problems, or had diabetes or other health conditions that can affect blood vessels in the eyes. It is not clear whether tadalafil causes vision loss.
Do not start or stop taking tadalafil during pregnancy without your doctor’s advice. Having pulmonary arterial hypertension during pregnancy could harm may cause heart failure, stroke, or other medical problems in both mother and baby. Tell your doctor right away if you become pregnant.
You should not breastfeed while using tadalafil.

Potential For Cialis To Affect Other Drugs
Aspirin Tadalafil did not potentiate the increase in bleeding time caused by aspirin.
Cytochrome P450 Substrates Cialis is not expected to cause clinically significant inhibition or induction of the clearance of drugs metabolized by cytochrome P450 isoforms. Studies have shown that tadalafil does not inhibit or induce P450 isoforms CYP1A2, CYP3A4, CYP2C9, CYP2C19, CYP2D6, and CYP2E1.
CYP1A2 Tadalafil had no significant effect on the pharmacokinetics of theophylline. When tadalafil was administered to subjects taking theophylline, a small augmentation of the increase in heart rate associated with theophylline was observed.
CYP2C9 Tadalafil had no significant effect on exposure to S-warfarin or R-warfarin, nor did tadalafil affect changes in prothrombin time induced by warfarin.
CYP3A4 Tadalafil had no significant effect on exposure to midazolam or lovastatin.
P-glycoprotein Coadministration of tadalafil for 10 days did not have a significant effect on the steady-state pharmacokinetics of digoxin in healthy subjects.

Females And Males Of Reproductive Potential
Infertility
Based on the data from 3 studies in adult males, tadalafil decreased sperm concentrations in the study of 10 mg tadalafil for 6 months and the study of 20 mg tadalafil for 9 months. This effect was not seen in the study of 20 mg tadalafil taken for 6 months. There was no adverse effect of tadalafil 10 mg or 20 mg on mean concentrations of testosterone, luteinizing hormone or follicle stimulating hormone. The clinical significance of the decreased sperm concentrations in the two studies is unknown. There have been no studies evaluating the effect of tadalafil on fertility in men .
Based on studies in animals, a decrease in spermatogenesis was observed in dogs, but not in rats .

What Other Drugs Will Affect Cialis

The efficacy and safety of Cialis for once daily use in the treatment of erectile dysfunction has been evaluated in 2 clinical trials of 12-weeks duration and 1 clinical trial of 24-weeks duration, involving a total of 853 patients.
