Do not use Kyleena during pregnancy. If left in place during pregnancy, the IUD could cause severe infection, miscarriage, premature birth, or death of the mother. The hormone in Kyleena may also cause unwanted effects in a female newborn.

Tell your doctor right away if you become pregnant. If you continue the pregnancy, watch for signs such as fever, chills, cramps, vaginal bleeding or discharge.

You should not use Kyleena if you are allergic to levonorgestrel, silicone, silica, silver, barium, iron oxide, or polyethylene, or if you have:

• abnormal vaginal bleeding that has not been checked by a doctor

• an untreated or uncontrolled pelvic infection

• endometriosis or a serious pelvic infection following a pregnancy or abortion in the past 3 months

• pelvic inflammatory disease , unless you had a normal pregnancy after the infection was treated and cleared

• uterine fibroid tumors or conditions that affect the shape of the uterus

• past or present cancer of the breast, cervix, or uterus

• liver disease or liver tumor

• a condition that weakens your immune system, such as AIDS, leukemia, or IV drug abuse

• if you have another intrauterine device in place

• if you had an abortion or miscarriage in the past 6 weeks or

• if you gave birth to a baby in the past 6 weeks.

Birth control pills are a form of hormonal contraception meant to prevent pregnancy. Most birth control pills contain the two hormones estrogen and progesterone. This helps block the release of eggs from the ovary, or ovulation. Some birth control pills, such as the minipill, help thicken cervical mucus to make it more difficult for sperm to reach an unfertilized egg.

Nope! Antibiotics like amoxicillin wont change the effectiveness of your birth control. The antibiotic rifampin is the only exception it can lower the effectiveness of the pill, patch, and ring. So unless youre using one of those birth control methods and taking rifampin, which is a medicine used to treat tuberculosis, theres nothing to worry about.

There are, however, some other medicines besides antibiotics that can make birth control not work as well. So its very important to be open and honest with your nurse or doctor about what medicines youre on no matter if it was prescribed or not so they can give you the best care.

Oral Contraceptive Steroid Pharmacokinetics

The estrogens most commonly found in oral contraceptive preparations are ethinylestradiol and mestranol, a prodrug which is metabolized to ethinylestradiol. After metabolism via the first pass effect, ethinylestradiol has an oral bioavailability of 40% to 50% . Hydroxylation is the main metabolic pathway for ethinylestradiol, whereas conjugation is considered to be a minor pathway in most women, resulting in sulphation or glucuronidation of the original estrogenic steroid. Glucuronide and sulphate conjugates reach the small intestine by way of the bile duct. Hydrolytic enzymes of intestinal bacteria break the conjugates down, resulting in the release of free, active estrogenic hormone. The active hormone is then available for reabsorption and undergoes enterohepatic cycling, which is responsible for plasma estrogen levels necessary for contraception.

The progestins present in oral contraceptive pills also undergo conjugation. Hydrolysis of conjugates leads to the formation of inactive metabolites because the parent molecule cannot be directly conjugated. Progestins are not thought to undergo extensive enterohepatic cycling and are, thus, less likely to be involved in drug interactions with antibiotics than ethinylestradiol .

Mirena Insertion And Removal

Women can have Mirena birth control inserted during a regular doctors visit. Mirena insertion takes about five minutes.

Doctors use a few tools to help insert the device but the insertion is not considered surgery.

Women should wait at least seven days after the start of their period to insert Mirena. Women who have just given birth, had a miscarriage or an abortion should wait at least six weeks.

A doctor or other trained health provider can remove Mirena at any time.

Eight out of 10 women who have had their Mirena IUD removed can get pregnant within a year, according to Bayers Mirena Handbook.

Health care providers must remove Mirena if it moves out of place. If a patient is not pregnant, she can get a new IUD inserted.

How To Prevent Birth Control Ineffectiveness

Always talk with your doctor about birth control interactions before taking new medications, including vitamins or over-the-counter medications. A 2012 meta-analysis published in the journal Human Reproduction looked at 42 studies and found that the copper IUD had a pregnancy rate of only 0.09% when it was used as a form of emergency contraception.

Of course, this isnt necessarily the most convenient form of emergency contraception. If you werent planning on getting an IUD, pill-based forms might make more sense for you depending on how easily youre able to access them versus an intrauterine device.

The Risk Of Expulsion And Perforation Are Very Low But They Do Occasionally Happen

IUDs come with a small risk of expulsion, which is when your body starts to push the IUD out of you. There isnt a ton of research on this out there, but the average expulsion incidence rate is between 2 to 10% of users, the ACOG says. Risk factors include recently having had the IUD inserted, getting it placed right after childbirth, and history of heavy or prolonged periods, among others. Though you dont really need to stress about this possibility, if it were to happen to you, you might experience symptoms like bleeding and cramping, feel something hard coming from your cervix, or see the IUD somewhere outside of your body however, it is possible to experience an expulsion without any major symptoms.

IUDs can also cause perforation, which is when the device pushes through the wall of your uterus. It sounds alarming, but again, the risk is really low. According to a 2015 study published in the journal Conception, out of 61,448 people who had an IUD, only 20 with the copper form experienced a perforation. In fact, perforation most often happens due to an error during placement. Much like with expulsion, this isnt something that needs to keep you up at night, but if you were dealing with perforation, you would potentially experience pelvic pain , though you might not experience any major symptoms at all.

The Copper Iud Is Hormone

Since the copper IUD doesnt contain hormones, you can use it in situations where other kinds of birth control may not be a fit. For instance, it might be a good choice for breastfeeding people, Taraneh Shirazian, M.D., assistant professor in the department of obstetrics and gynecology at NYU Langone Medical Center, tells SELF. This is because theres a slight chance that estrogen, which is in forms of combined hormonal birth control like the pill, patch, and ring, may impact your milk supply, according to the ACOG.

Due to its lack of estrogen, the copper IUD may also offer an advantage for some people who have health conditions that can get worse when exposed to this hormone, such as people with a history of blood clots, or high blood pressure , Dr. Shirazian says.

If youre not able to tolerate hormones for some reason, definitely mention that to your doctor when trying to find which type of IUD is right for you.

What If My Healthcare Provider Prescribes Rifampin For Me

If your healthcare provider does prescribe rifampin to you, be sure to let them know if you are using a hormonal contraceptive.

Unless otherwise stated by your healthcare provider, you can still continue to take your pills as usual every day. However, during this time, itâs important to use non-hormonal birth control methods, like external or internal condoms, or abstain from sex as backup protection during rifampin treatment .

If you use birth control pills, consider following the recommendations for what to do if you miss two or more pills:

• Continue to use these non-hormonal backup methods for 7 days after you stop taking rifampin, provided you still have 7 hormone-containing pills left in your pack

• If there are fewer than 7 hormonal pills left in the pack after you stop taking rifampin, skip the hormone-free pills and start a new pack, but still continue to use back-up contraception for the first 7 pills of the pack .

If you are using another form of hormonal contraceptive, like the patch or ring, speak to your healthcare provider.

How Does Mirena Removal Affect Fertility

Most womens fertility will go back to normal after Mirena removal, and they can get pregnant quickly. If a woman does not want to get pregnant after having an IUD removed, she should use another form of birth control.
